This is an application to umpire in the Boys Baseball, Inc. program. No individual is eligible to participate in any capacity without approval from the Board of Directors. The Screening Committee will not consider any application unless all questions are answered. You may be accepted or rejected due to the information on this form; your past actions both on and off the diamonds; or any other information developed by the committee, including their opinion of your knowledge of both baseball and B.B.I. rules and your ability to control a game. Acceptance or rejection is for one year only.

I am fully aware of the risks incurred by baseball umpires. I agree that Boys Baseball, Inc., its directors, officers, sponsors, employees, managers and coaches, shall not be liable for any injury, loss or damages that may occur as a result of my participation as an umpire.

I also understand and agree that various background inquires may be made by B.B.I., specifically including inquires to law enforcement and security agencies. I acknowledge the right of B.B.I. to make said background inquires generally and law enforcement and security agency inquiries specifically, and I expressly waive any and all claims to privacy with respect to records and information pertaining to me which any agency may possess. i certify that all statements made by me are true and without reservation. If anything contained in this application is found to be untrue, I understand I will not be accepted or, if already accepted, I understand I will be subject to immediate suspension.

I understand that $5.00 will be deducted from my first check to cover the expense of this application and I am not subject to backup withholding.
